2.Type of ban (Perma,Ip,Temp [how long])

3.Who banned you (try to think which staff members was on at that time)

4.Reason you were banned (be honest we will find out if you are lying)

5.Do you admit you have done anything wrong

6.Why you should be unbanned

Just Because You Make an Appeal Does Not Guarante You Will Be Unbanned
